          The public health system in Buffalo, Dawson, Franklin, Gosper, Harlan, Kearney and Phelps Counties is participating in a Mobilizing for Action through Planning and Partnership Assessment (MAPP) to identify priority health issues in this area and develop plans and partnerships to address them.  We would like to ask you to participate in an anonymous Community Health Survey designed to gather public opinion on health related issues within the Two Rivers Public Health District.  Your responses to this survey will play a vital role in identifying health issues that affect people’s lives throughout the district.  Please share your opinions and help us to identify and improve existing services, and develop plans to address community health needs.
