Grant Information

Grant funding is available from Two Rivers Public Health Department to request funds for educational programs related to public health or matching funds for health related programs.  Requests for funds cannot exceed $5000.00 per application.  The Two Rivers Board of Health Grant Review Committee will evaluate the requests and make a recommendation on funding to the Board of Health.  The project must address the public health priority areas identified by the Two Rivers Public Health Department Board of Health which can be found on our website.  The decision on the funding request will be made at the Board of Health meeting following submission of the request.  All forms are available from our office or please click on links below to download Grant Information online.
